Please read the information below before clicking on the referral link. This page is only for Approved Education Professionals to make referrals. NB - Any referrals not from an authorised professional will not be accepted.
To support you in completing the referral, you will need to have all the required information relating to the CYP you are referring before accessing the link.
Please note, the form cannot be partially saved, so you will need to complete and submit the referral in one go.
It is important that before submitting the referral for a child or young person, that you share Noah's Ark Centre's Data Privacy Notice (click here to view and download) with a person with parental responsibility or the young person themselves if they are over 15 as you will be asked to confirm that you have done this when making the referral
Please ensure that you select the correct option (see below) from the drop-down list in the field “Which service are you referring to?” as this may delay the referral process.
Referral forms must be completed at least 2 working days prior to a CYP seeing a therapist.
Once a referral has been submitted, please notify the allocated therapist (s) delivering sessions in your school.
